Please read and follow these steps BEFORE clicking on the Apply to Be A Food Vendor form. (Having these files filled out and ready for upload will make your application process faster. Click on the Images below to download the files.)

1. Read this Temporary Food Vendor Application packet - required by City of Lowell

2. Fill out the City's Temporary Food Vendor 2 page application and Employee Log (pages 4 - 6 of the Packet) and submit to our online form. (DO NOT MAIL OR SUBMIT TO THE CITY HEALTH DEPARTMENT!)

3. Have a pdf or jpeg of your valid Servsafe Certificate (not required for Plant/Produce Food Vendors)

4. Have a pdf or jpeg of your valid Allergen Certificate (not required for Plant/Produce Food Vendors)

5. Fill out Fire Permit application (you can do this at time of application, or hold off until after you are a confirmed Food Vendor. Fire permit fees will not be processed until selected. (Submit by uploading in our form, do not mail or submit directly to the Fire Department.)

6. AFTER PREPARING ALL THESE FILES AND DOCUMENTS, fill out the Festival's Food Vendor Application and Contract Form.  Don't forget to upload files mentioned in #2 and #6.)

NOTE: Submitting an application does not GUARANTEE Participation. You will be informed on June 1st of your selection status.

Due to the large increase in the number of applicants and inquiries, we will no longer accept texts of these required documents. Please upload all files in the application form (via the Green "Apply to Be a Food Vendor" button.)
